Contact Tracing and Care Coordination for Vulnerable Populations through the Central Ohio Pathways HUB
Collaboratives across the country are taking swift action to support their key stakeholders in responding to the emergent community needs resulting from the coronavirus. The Network for Regional Healthcare Improvement (NRHI) is honored to share key findings, promising practices, important innovations, and stories of hope from our members and other national thought leaders as we all grapple with a path forward. Join us for a series of webinars intended to foster shared learning, problem solving, and increased connections.

We are stronger together!

Join us for the next webinar of our series, where we will hear from NRHI member, the Healthcare Collaborative of Greater Columbus. They will share about their partnership with a local health department in providing contact tracing and care coordination for citizens who speak English as a second language. The focus will be on using community health workers' unique position in the communities in which they serve to provide trusted, reliable contact tracing and care coordination services, ensuring that citizens are provided with sustainable connections to care and services even after the contact tracing process is completed.
